Hypertension
- If diastolic
blood pressure (DBP) > 120 mmHg or systolic blood pressure (SBP) >
220 mmHg:
- Nitroprusside
0.5-5 mcg/kg/min until:
- DBP
decreases by 20% or
- SBP
< 180 and DBP < 120
- If DBP
100-120 or SBP 180-220 mmHg
- Labetolol
10 mg IVP over 1 min with repeated doses doubling the dose every 10
minutes up to 4 times (10, 20, 40, 80)
- If still
uncontrolled, can start labetolol infusion 2-8 mg/min
- If
patient has COPD, asthma, or bradycardia (must avoid beta blockade)
- Hydralazine
20 mg IVP every 15 minutes until SBP 140-160
- If resistant,
start nitroprusside drip
